- Company name and your return shipping address (No P.O. Boxes)
- Technician's contact name and phone number
- A technical description of the problem
- Please pack your unit to protect it from damage during shipping, and insure it for its replacement value

- TPL Communications does not cover the shipping cost to ship an RMA to us.
- We recommend using a traceable shipping method (Federal Express, UPS, DHL, etc.) TPL Communications is not responsible for any lost or damaged items during shipment to the service center.
- The warranty on your repaired amplifier is 90 days and is limited to cover the original repair only.
